Diagnostic Automation 17-Hydroxyprogesterone (17-OH progesterone or 17OHP) is a C-21 steroid hormone
produced in the adrenal gland and gonads, during the synthesis of glucocorticoids and sex steroids. It is derived
from progesterone via 17-hydroxylase, a P450c17 enzyme, or from 17-hydroxypregnenolone via 3β-hydroxysteroid
dehydrogenase/Δ5-4 isomerase.
17α -OHP has no defined physiologic role except as a precursor molecule.
Serum 17α -OHP levels are age-dependent, with peak levels observed during fetal life and the immediate postnatal
period. During the first week of life, serum 17α -OHP level

The steroid 17-α-Hydroxyprogesterone (17-α-OHP) is produced by both the adrenal cortex and gonads. Even though 17-α-OHP has relatively little progestational activity, it is of intense clinical interest because it is the immediate precursor to 11 -desoxycortisol (Cpd-S). Because Cpd-S is produced by 21 -hydroxylation of 1 7-α-OHP, measurement of 1 7-α-OHP is a useful indirect indicator of 21 -hydroxylase activity. In congenital 21 -hydroxylase deficiency, the most common variety of congenital adrenal hyperplasia (CAH), 17-α-OHP is secreted in abundant excess. It is moderately elevated in the 1

The Progesterone (Salivary 17-OH) EIA is a solid phase enzyme-linked immunosorbent assay (ELISA),based on the principle of competitive binding. The microtiter wells are coated with a polyclonal antibody directed towards an antigenic site on the 17α-OHP molecule. Endogenous 17α-OHP of a sample competes with a 17α-OHP-horseradish peroxidase conjugate for binding to the coated antibody. After incubation the unbound conjugate is washed off. E3 The amount of bound peroxidase conjugate is inversely proportional to the concentration of 17α-OHP in the sample. After addition of the substrate solution, t

17 a-hydroxyprogesterone is a steroid hormone with a molecular weight of 330.4.Its assay enables the establishment of an early diagnosis and the adaptation of the treatment of congenital hyperplasia of the adrenal glands. This recessive autosomal hereditary condition is characterized by an enzymatic deficiency in the biosynthesis of cortisol.
